Physical Examination

Students registering for Athletics must have a current physical examination form submitted for the Athletic Department review before the student is authorized to participate in practice or tryouts. The Physical Examination shall not be taken earlier than May 1 preceding the school year for which it is applicable. Download the physical form by clicking the Athletic Team Registration icon and complete a family account and follow the download instructions.

Physical exams are required annually dated after May 1st.
